people are being sent down into the precarious labyrinthine brain of a vast buried monster. they are unable to harm it for Reasons but they can manipulate the fatty folds of tissue a bit. it is dryer and firmer than exploring a "real" giant brain but there's definitely Organic Stuff happening in there. and having reached a deeper layer, sometimes they find a player character! just curled up in the dark, dreaming about being buried alive with a headache.
the pcs are manifestations of the creature's dreams. this doesn't mean they/their canons are only a dream (but like if you wanted to play it for some existential stuff you could) so much as it is so powerful and otherworldly that its dreams reach across universes. dropping or hiatusing feels like waking from a dream.
the local surface civilization is pseudoroman. perceiving the creature as a cthonic god, having festivals that they hope will appease it, and having an empire that is very invested in the creature either not waking up or being controlled if it does wake up, but has limited means compared to modern or futuristic society. unfortunately for them they can't seem to harm it. the entry orifice (wound?) has been here for as long as anyone's been able to check. its fluids permeate the ground, plants grow here that don't grow anywhere else, strange moods take people, hate wisps and monsters sometimes come out of the hole, there's an aqueduct for regular water but if/when tremors break it, well (well).
the main thing people are doing is trying to remove the hateful manifestations and plant more benevolent seeds harvested from their own memories. there's an ongoing debate about if the hate wisps colonized it from the outside, or it's the source of them. there are some dicey ethics about trying to literally change its mind but they've got a lot at stake. lots of the people in 'town' are not here voluntarily but by the will of the empire and may not all be complying in good faith.
pcs would be better able to affect the creature and could kill it eventually, but is that a good idea? they might also be able to effectively keep it from waking indefinitely and leave the surface people to just guard the opening. otherwise, the creature is going to wake. it stirs and the ground shakes and some of these eyeball palms rise up from the ground, watching vaguely. this game doesn't run indefinitely.

there's space for memory share elements starting with donating good memories to be planted(characters don't forget them dw) though 'good' has a lot of flex in it, it doesn't have to be just happy peaceful stuff. those are voluntary, there may be involuntary ones that are worse. there would also be space for weirdness and creative events.
