Unfortunately the only time you can ask Krem about physical transitioning is in the context of Blood Magic, and he gives a very hard NO because Blood Magic is fucked up and chancy at best he doesn't want that anywhere near his body. There's no option to ask about physical transitioning via other means, so it is left up to individual interpretation.
